Total Care Team is dedicated to providing professional, person-centred support that empowers NDIS participants to achieve their goals. Our experienced team brings together diverse qualifications across Allied Health, Disability, and Aged Care, offering tailored assistance in Support Coordination, Social Work, Occupational Therapy, and Allied Health Assistance.

Total Care Team is offering Specialist Support Coordination (Level 3), Support Coordination (Level 2), Psychosocial Recovery Coaching, Occupational Therapy, Social Work & Counselling, and Therapy Assistance. We support Agency, Plan, and Self Managed participants, as well as clients under Home Care Packages, Restorative Care, TAC, Aged Care, and Employment Assistance Programs.

Based in Barwon South West Victoria, we assist participants across Warrnambool, Port Fairy, Portland, Hamilton, Terang, Camperdown, Colac, Koroit, Horsham, Stawell, Geelong, and surrounding areas. Our services are also available Australia-wide via Microsoft Teams, Zoom, and phone consultations, providing flexible and accessible support wherever you are.
